Financial Policies

Dr. Ben Moosavi and his dedicated team at ULTIMATE Plastic & Reconstructive Surgery represent a rare and exceptional standard in cosmetic surgery. As one of the few double board-certified plastic surgeons with a focused expertise in facial and neck procedures, Dr. Moosavi consistently delivers natural, transformative results in face, breast and body rejuvenation. Dr. Ben Moosavi and his team are committed to delivering the highest level of surgical care using advanced techniques, all while prioritizing patient safety, satisfaction, and a seamless experience. To uphold these standards, we have established the following financial policies:

 

Reservation Fee – Cosmetic Procedures Only

A $500 reservation fee is required at the time of booking any cosmetic surgical procedure. This amount will be applied toward the total cost of your surgery.
The fee is fully refundable if written cancellation notice is provided at least 21 days in advance of your scheduled procedure date.
Please note: This reservation fee policy applies only to elective, self-pay cosmetic procedures and does not apply to procedures involving insurance coverage.

Payment Policy – Cosmetic Procedures Only

Full payment for all cosmetic surgical procedures is due no later than two weeks prior to your scheduled surgery date.
Accepted forms of payment include cash, check, and credit card.
Please note: If payment is not received by the deadline, your procedure will be canceled, and applicable cancellation fees will be incurred.
This payment policy applies exclusively to elective, self-pay cosmetic procedures and does not apply to any procedures billed through insurance.

Please note: Surgical facility fees and anesthesia are billed separately by the hospital or surgical center and must be paid directly according to their individual policies.

Surgery Rescheduling Policy

Procedures rescheduled with less than 3 weeks’ notice will incur a $100 rescheduling fee.

Please note: If pre-operative instructions are not followed—such as avoiding nicotine, taking or avoiding medications as directed, or adhering to day-of-surgery protocols—your surgery may be canceled. In such cases, rescheduling and cancellation fees will still apply.

Cancellation Policy 

We require at least 21 days’ notice for surgery cancellations due to non-medical reasons. Cancellations made 21 days of the scheduled surgery date are not eligible for a refund.  If surgery is canceled on the day of the procedure–or if pre-surgical instructions were not followed–all funds paid toward the surgery will be forfeited. If adequate notice is not provided, the $500 reservation fee will be forfeited. To cancel, please call our office Monday through Friday to speak with our scheduling coordinator directly. A new surgical date will not be confirmed until all outstanding fees are paid.
