Financial Policies
Dr. Ben Moosavi and his dedicated team at ULTIMATE Plastic & Reconstructive Surgery represent a rare and exceptional standard in cosmetic surgery. Please note: Surgical facility fees and anesthesia are billed separately by the hospital or surgical center and must be paid directly according to their individual policies.

Surgery Rescheduling Policy
Procedures rescheduled with less than 3 weeks’ notice will incur a $100 rescheduling fee.
Please note: If pre-operative instructions are not followed—such as avoiding nicotine, taking or avoiding medications as directed, or adhering to day-of-surgery protocols—your surgery may be canceled. In such cases, rescheduling and cancellation fees will still apply.
Cancellation Policy
We require at least 21 days’ notice for surgery cancellations due to non-medical reasons. Cancellations made 21 days of the scheduled surgery date are not eligible for a refund. If surgery is canceled on the day of the procedure–or if pre-surgical instructions were not followed–all funds paid toward the surgery will be forfeited. If adequate notice is not provided, the $500 reservation fee will be forfeited. To cancel, please call our office Monday through Friday to speak with our scheduling coordinator directly. A new surgical date will not be confirmed until all outstanding fees are paid.
